AI-PV時代のPV組織を再設計する
― PV専門家は、もはや不要になるのか?

AIがCase Intake、AE情報抽出、MedDRA Coding、Narrative作成、Literature Screening、Duplicate Detection、さらにはSignal Detectionまで支援する時代になりつつあります。

では、AIによってPV業務が効率化されれば、PV部門の人員を減らすことができるのでしょうか? そして将来、PV専門家は不要になるのでしょうか?

私は、問題はそれほど単純ではないと考えています。AIによって定型的な処理業務が減る一方で、PV部門には新しい役割が生まれます。

  • AIが処理できなかった例外症例の評価
  • AI出力の医学的妥当性の判断
  • False Negativeが患者安全へ与える影響の評価
  • SignalのValidationとBenefit-Risk Assessment
  • AI性能の継続的な監視
  • モデル、データ、業務プロセスの変更管理
  • AIを使用した判断についての説明責任

つまり、人間の仕事がなくなるのではなく、「症例を処理する仕事」から「AIの結果を評価し、安全性上の意味を判断する仕事」へ、PV専門家の役割が移っていく可能性があります。

一方、これまで若手PV担当者は、一件一件の症例を読み、Coding、Narrative、QC、Medical Reviewを経験しながら、PV判断の基礎を身につけてきました。AIがその入口を代替した場合、次世代のPV専門家は、どこで症例を読む力を身につけるのでしょうか?

さらに、誰がAIの使用範囲を決め、性能低下を判断し、使用を止め、最終的な説明責任を負うのか。PV、IT、QA、ベンダーの間に新しい責任分界も必要になります。

AI-PVがもたらす最大の変化は、単なる業務効率化ではありません。これまでのPV組織、人材配置、育成経路、そして「PV専門性とは何か」を、そのまま維持できなくなることではないでしょうか。

このシリーズでは、業務の再配分、PV専門家の育成、部門間の責任分界、新しいPV組織と人材配置という観点から、全5話で考えます。次回は「AIによって失われる仕事と、新たに生まれる仕事」です。

What Will AI-PV Bring to the PV Organization?  < Episode 1>

AI is beginning to support case intake, adverse-event extraction, MedDRA coding, narrative drafting, literature screening, duplicate detection and even signal detection.

If AI makes PV operations more efficient, can the PV workforce simply be reduced? Will PV professionals eventually become unnecessary?

I do not think the answer is that simple. As routine processing declines, new responsibilities emerge within the PV organization.

  • Assessing complex and exception cases that AI cannot resolve
  • Judging the medical validity of AI outputs
  • Evaluating the patient-safety impact of false negatives
  • Signal validation and benefit-risk assessment
  • Continuous monitoring of AI performance
  • Managing changes to models, data and processes
  • Accountability for decisions made with AI support

The human role may therefore shift from processing cases to evaluating AI outputs and determining their safety significance.

There is also a development challenge. Traditionally, junior PV professionals learned by reading cases, coding, drafting narratives, performing QC and progressing to medical review. If AI takes over that entry path, where will the next generation learn to read and interpret cases?

New accountability boundaries are also needed. Who defines the scope of AI use, identifies deterioration, stops the system and carries final accountability across PV, IT, QA and vendors?

The largest change brought by AI-PV may not be efficiency. It may be that we can no longer preserve the existing PV organization, workforce allocation, development pathway and definition of PV expertise unchanged.

This five-part series examines work redistribution, expert development, accountability and a future PV organization model. Next: “Work that declines—and work that emerges.”

AI-PV時代のPV組織を再設計する
― PV専門家は、もはや不要になるのか?

AIをPV業務へ導入するとき、経営側から最初に問われるのは、「何時間削減できるのか」「何人分を代替できるのか」「外部委託費をどこまで削減できるのか」という点でしょう。もちろん、生産性向上や経費削減は重要です。

しかし、AI-PVの効果をFTE削減だけで評価すると、PV組織に必要な能力まで失う可能性があります。

  • 縮小する仕事:手入力・転記、定型的なAE情報抽出、定型Narrative、一律的QC、RoutineなLiterature Screening、単純集計
  • 比重が高まる仕事:複雑・例外症例、医学的整合性、Signal Assessment、Benefit-Risk、Safety Communication、AI性能・Data Quality・Drift監視

仮にAIがRoutine Caseの80%を処理できたとしても、残る20%は従来より複雑で、判断の難しい症例に集中するかもしれません。さらに、AIの管理、監視、説明という新しい業務も加わります。

したがって、AI-PV時代の要員計画で問うべきなのは「何人減らせるか」だけでなく、「AIによって生まれた時間と人材を、どの安全性業務へ再配置するか」です。

Case Processingで削減された工数を、Signal Management、Aggregate Assessment、RMP、Benefit-Risk、Safety Communication、AI-PV Oversight、次世代PV専門家の育成へ移すことが考えられます。

AI-PV導入は、PV部門を縮小する計画ではなく、「処理中心の組織」から「安全性を評価し、判断する組織」へ人的資源を再配分する機会ではないでしょうか。

Work That Declines—and Work That Emerges

Redesigning the PV Organization in the Age of AI
— Will Pharmacovigilance Professionals Still Be Needed?

When AI is introduced into PV, management is likely to ask first: How many hours can be saved? How many FTEs can be replaced? How much outsourcing cost can be reduced? Productivity matters.

But if AI-PV is evaluated only through FTE reduction, the organization may also lose capabilities it still needs.

  • Work likely to decline: data entry, transcription, routine extraction, standard narratives, uniform QC, routine literature screening and simple aggregation
  • Work likely to grow: complex and exception cases, medical consistency, signal assessment, benefit-risk, safety communication, AI performance, data quality and drift monitoring

Even if AI processes 80% of routine cases, the remaining 20% may contain a greater concentration of complex, judgment-intensive work. New activities for managing, monitoring and explaining AI will also appear.

The workforce question should therefore not be only “How many people can we reduce?” It should be “Where should we redeploy the time and capabilities released by AI?”

Capacity released from case processing can be redirected to signal management, aggregate assessment, RMP, benefit-risk, safety communication, AI-PV oversight and expert development.

AI-PV should be treated as an opportunity to move human capability from a processing-centered organization to one centered on evaluating and making safety decisions.

症例処理を経験しないPV専門家を、どう育てるのか?

AIがAEを抽出し、MedDRA Termを提案し、Narrativeを作成し、重篤性や報告要否まで支援する。この仕組みはPV業務を大きく効率化します。

しかし、若手PV担当者は、どこで症例を読む力を身につけるのでしょうか?

これまでPV専門家は、Case IntakeからCoding、Narrative、QC、Medical Review、Aggregate Assessment、Signal Management、RMP・Benefit-Riskへと経験を重ねてきました。多数の症例を読み、判断を間違い、先輩から修正される中で、「何か気になる」という感覚を身につけてきました。AIが入口業務を担う環境では、次のような状態が生じやすくなるかもしれません。

  • AIの誤りに気付けないReviewer
  • 原資料を読まず、AIの要約だけを見る担当者
  • MedDRA Termと臨床像を結び付けられない担当者
  • Confidence Scoreを医学的確信度と誤解する担当者
  • Dashboardは読めても、個別症例へ戻れないSignal Assessor

AIが初級業務を代替すると、作業負担だけでなく、この学習機会まで失われる可能性があります。AI時代には、OJTを自然発生に任せることができません。

AI出力を見ずに症例を評価する、AIと人の判断差をレビューする、False Positive/False Negativeを教材化する、原資料からICSR完成まで経験する、Signalから個別症例へ遡るなど、意図的な学習設計が必要です。

Case Processing、Medical Review、Signal Management、Aggregate Reporting、RMP、PV Quality、AI-PV Oversightをつなぐジョブローテーションも重要になります。これはGeneralistを量産するためではなく、自分の専門領域と前後の安全性判断プロセスを理解できる専門家を育てるためです。

AI-PV時代の大きなリスクの一つは、AIの性能低下だけではなく、「AIを正しく評価できるPV専門家が、将来いなくなること」なのかもしれません。

How Do We Develop PV Experts Who No Longer Learn Through Case Processing?

— Will Pharmacovigilance Professionals Still Be Needed?

AI can extract events, suggest MedDRA terms, draft narratives and support seriousness or reportability decisions. This can transform efficiency.

But where will junior PV professionals learn to read cases?

PV experts traditionally progressed through intake, coding, narratives, QC, medical review, aggregate assessment, signal management, RMP and benefit-risk. By reading many cases, making mistakes and receiving expert correction, they developed the ability to sense when something was not quite right. Where AI performs that entry-level work, the following may become more common:

  • Reviewers unable to recognize AI errors
  • Staff who read only AI summaries, not source information
  • Staff unable to connect MedDRA terms with clinical reality
  • Confusion of confidence scores with medical certainty
  • Signal assessors who can read dashboards but cannot return to individual cases

If AI replaces entry-level work, it may remove not only workload but also learning opportunities. OJT can no longer be left to happen naturally.

Organizations may need deliberate learning: assess cases before viewing AI output, review AI-human disagreement, use false positives and false negatives as training cases, follow source information through the completed ICSR and trace signals back to cases.

Rotation across case processing, medical review, signal management, aggregate reporting, RMP, PV Quality and AI-PV oversight also becomes important—not to produce generalists, but specialists who understand how their work connects to the safety process around it.

A major AI-PV risk may not only be declining model performance. It may be that, in the future, too few PV professionals remain capable of evaluating AI correctly.

PV・IT・QA・ベンダーの境界をどう再設計するか?

AI-PV時代のPV組織を再設計する
― PV専門家は、もはや不要になるのか?

従来のPVシステムでは、PV部門は業務、ITはシステム管理、QAは独立的確認、ベンダーは製品と技術を提供する、という比較的分かりやすい役割分担がありました。

しかしAI-PVでは、この分担だけでは十分でない場合があります。AIの使用業務、自動処理対象、Confidence Threshold、Human Escalation、False Negativeの許容水準、性能低下時の制限・停止、モデル更新後の継続使用は、技術問題であると同時に患者安全上の判断です。

  • PV:Intended Use、患者安全リスク、医学的性能基準、Escalation、使用継続・制限・停止
  • IT/Data:技術基盤、Integration、Security、Data Pipeline、技術的変更
  • QA/CSV:独立的保証、適合性、監査・査察、逸脱・CAPA
  • Vendor:製品性能、技術情報、更新情報、障害・変更の通知

ITやベンダーはPrecision、Recall、F1 Score、Driftを説明できます。しかし、その数値が重篤症例の見落とし、規制報告の遅延、新しいSignalの発見遅延として何を意味するかは、PV部門が判断しなければなりません。

一方、PV部門だけでAIモデル、データ品質、変更管理、Cybersecurityをすべて評価することも困難です。そのため、PV、Safety Science、IT/Data、QA/CSV、Legal、Procurement、Vendorをつなぐ横断的な「AI-PV統合推進機能」が必要になります。

これは必ずしも新しい独立部署を意味しません。小規模なCenter of Excellence、Steering Team、あるいは兼務機能でも構いません。重要なのは、PV要件をAI技術へ、技術指標を患者安全上の意味へ変換する責任が明確であることです。

AIの技術管理はITやベンダーへ委ねられても、「そのAIをPV業務に使用してよいか」という判断と説明責任まで委ねることはできません。

How Should We Redesign the Boundaries Between PV, IT, QA and Vendors?

Traditional PV systems often had relatively clear boundaries: PV operated the process, IT managed the system, QA provided independent assurance and vendors supplied products and technical services.

With AI-PV, those boundaries may no longer be sufficient. Scope of use, automated-case eligibility, confidence thresholds, human escalation, acceptable false-negative levels, restrictions or shutdown and post-update continuation are technical questions and patient-safety decisions at the same time.

  • PV: intended use, patient-safety risk, medically meaningful criteria, escalation and use/limit/stop decisions
  • IT/Data: platform, integration, security, data pipelines and technical change
  • QA/CSV: independent assurance, compliance, audit/inspection, deviations and CAPA
  • Vendor: product performance, technical information, updates, incidents and change notification

IT and vendors can explain precision, recall, F1 and drift. PV must determine what those metrics mean as missed serious cases, delayed regulatory reporting or delayed signal identification.

At the same time, PV alone cannot evaluate models, data quality, change control and cybersecurity. A cross-functional AI-PV Integration Office is therefore needed to connect PV, Safety Science, IT/Data, QA/CSV, Legal, Procurement and vendors.

This does not necessarily mean a new department. It can be a small center of excellence, steering team or assigned cross-functional capability. What matters is clear ownership for translating PV needs into technical requirements and technical metrics into patient-safety meaning.

Technical management may be delegated to IT or a vendor. The decision to use AI in PV—and accountability for its use—cannot be delegated away.

AI-PV時代のPV組織を再設計する
― PV専門家は、もはや不要になるのか?

これまで、定型業務と専門業務の再配分、症例処理を通じたOJT機会の減少、PV専門家の意図的な育成、PV・IT・QA・ベンダーの責任分界を考えてきました。

では、AI-PV時代のPV組織は、どのような形になるのでしょうか。私は、少なくとも次の4つの中核機能と、それらをつなぐ1つの横断機能を明確にする必要があると考えます。

  • PV Operations:AI支援Case Processing、Regulatory Reporting、例外症例、委託先を運営する
  • Safety Science:Medical Review、Signal、Aggregate、RMP、Benefit-Risk、Safety Communicationを担う
  • AI-PV & Data Oversight:AI Use Case、性能、Data Quality、Drift、モデル変更、Human Escalationを管理する
  • PV Quality & Capability:独立的保証、SOP、監査・査察、Competency、教育、ジョブローテーションを担う
  • AI-PV統合推進機能:PV・Safety Science・IT/Data・QA・Vendorを横断し、要件・指標・患者安全上の意味をつなぐ

大規模企業では専門チームとして設けられますが、中小規模の製薬会社では独立部門を増やす必要はありません。複数の役割を兼務しても構いません。重要なのは組織名ではなく、誰が日常運用、医学的判断、独立的保証、使用制限・停止、次世代育成を担うかが明確であることです。

また、基礎段階ではCase Processingと原資料、専門段階ではMedical Review、Signal、Aggregate、RMP、Quality、統合段階ではAI-PV Oversight、Data Governance、Vendor Management、Inspection対応を経験する、段階的なジョブローテーションが考えられます。

AI-PV時代にPV専門家が不要になるわけではありません。しかし、定型業務を処理することだけを専門性としてきた役割は縮小するでしょう。

これから必要になるのは、AIより速く処理する人ではありません。AIが何を見落とし、その結果が患者安全に何を意味するかを判断できるPV専門家です。

AI-PV時代のPV組織再設計とは、人をAIに置き換える計画ではありません。人にしか担えないPV専門性を、組織の中心に置き直す計画なのだと思います。

A Future PV Organization and Workforce Model

Redesigning the PV Organization in the Age of AI
— Will Pharmacovigilance Professionals Still Be Needed?

We have considered redistribution of routine and expert work, the loss of natural OJT through case processing, deliberate expert development and new accountability boundaries across PV, IT, QA and vendors.

What might the PV organization look like in the age of AI? I believe at least four core functions—and one integrating capability—must be made explicit.

  • PV Operations: AI-assisted case processing, reporting, exceptions and service-provider oversight
  • Safety Science: medical review, signals, aggregate assessment, RMP, benefit-risk and safety communication
  • AI-PV & Data Oversight: use cases, performance, data quality, drift, model change and human escalation
  • PV Quality & Capability: independent assurance, SOPs, audit/inspection, competency, learning and rotation
  • AI-PV Integration Office: connects PV, Safety Science, IT/Data, QA and vendors, translating requirements, metrics and patient-safety meaning

Large organizations may create specialist teams. Smaller companies do not need to add separate departments; roles may be combined. What matters is clarity about who owns operations, medical judgment, independent assurance, restriction or shutdown and development of the next generation.

A staged rotation can combine foundational experience in cases and source information, specialist experience in medical review, signals, aggregate reporting, RMP and Quality, and integrative experience in AI-PV oversight, data governance, vendor management and inspection readiness.

PV professionals will not become unnecessary. Roles whose expertise consists only of routine processing, however, are likely to shrink.

The future does not need people who process faster than AI. It needs PV professionals who can identify what AI has missed and determine what the result means for patient safety.

Redesigning the PV organization is not a plan to replace people with AI. It is a plan to put uniquely human PV expertise back at the center of the organization.
